The referral-based program provides preventative care and fosters self-management through enhanced education and supports that have the potential to significantly improve health outcomes and quality of life for those living with Chronic Obstructive Pulmonary Disease (COPD) and asthma. Services are provided by a registered respiratory therapist (RRT) who is also a certified respiratory educator who works in collaboration with the patient’s primary care provider to treat the patient.
